| Required for both athlete and parent/guardian to complete each year after viewing the Code of Conduct video. Paper forms may be found in the middle school office. |

Concussion and Sudden Cardiac Arrest Waiver
|
| Required for athlete and parent/guardian to complete each year. The waiver to complete should be done through the Annual Update online. Visit your Parent Portal; under the More section, complete the Annual Update registration. Be sure to say "Yes" to the question asking if your student is participating in athletics. Paper forms can also be found in the middle school office. |

| Required for a parent to complete when transporting an athlete to or from a game or event. The form must be completed and turned in to the coach on the day of the event. Please see the form for more details. |
