Eligibility Requirements - Red Cards

In order for a middle school student to be eligible to start a season, they must have all required Red Card items turned into middle school office.  Students will not be able to practice until all required items are received.  If you are not sure of your last physical, please contact Justine Ley.  Physical appointments can be difficult to schedule and are required to participate.  Please do not wait until the last minute to complete this requirement.  Physical forms are available online through the middle school website under Athletic Forms and also available in the middle school main office. 

Please note items with one * should be completed  online via the Annual Update/Online Student Registration for the 2026-2027 school year  via the Athletic Tab.  

  1. WIAA Physical - physical sign by a physician at school is required every two years
  2. Emergency Contact Information * - completed via the OLR Annual Update found on your Parent Portal under the More section.  Please be sure to complete the Athletics portion of the OLR registration.
  3. Concussion and Sudden Cardiac Arrest Waiver * - completed via the OLR Annual Update found on your Parent Portal under the More section.  Please be sure to complete the Athletics portion of the OLR registrations. Forms can also be available in the middle school office. 
  4. Athletic Fee Payment - Athletic fee is $30.00 per sport and can be paid online or in the middle school office
  5. Code of Conduct Video and Acknowledgement - Student and parent  must  view video and form must be signed annually by both student and parent
